Your first visit in our office will take about an hour. You will need to complete paperwork that will give us information about you as well as health history and current symptoms. If you wish to save time, please click here *Welcome Form* and *Health Questionnaire* to download and complete the paperwork to bring to your appointment.

After that is completed, Dr. Slavik will meet with you to get some additional information and discuss whether chiropractic is an option for you.

Then you will be given a complete exam and x-rays, if necessary.

After reviewing the exam and x-ray information, Dr. Slavik will set up a time to review his findings or begin treatment if that is appropriate in your case.
